The information submitted below will be used to create your facility’s account within our Smart Therapy™ Complete Solution platform and the tablet’s application. It is inside your account where all of your devices, patient profiles, and data will be stored. Upon account creation and delivery of the system, your registered email below will be provided with a login and password to access the account.

It is important to note that all emails detailing the onboarding and training process, as well as account login information, and notifications of day and time for future updates will be sent to the email used below, tied to the registration of your account. Preferably use the email and phone number of your therapy director and/or end user point of contact for implementation going forward.
