Documenting and billing for treatment within occupational and physical therapy can be the only downside of the job: It takes us away from the joy of treating our patients and isolates us behind a desk. Even if documenting, to put it simply, is writing “the story” of what happened during our treatment, we unfortunately are limited by vocabulary leaving our notes far from literary works as colorful as our favorite novels.
